New Client Packets
Immediately below are new client packets for both adults and children. You may download these packets and bring them completed to your first visit, or arrive fifteen minutes prior to your scheduled appointment and we will provide a packet for you to complete. Our New Client Packets are available for both children and adults and contain all forms needed to begin therapeutic services at The Therapy Tree.
Most if not all of our forms and surveys can be found on this page. Please print and complete the necessary forms to bring with you on your first appointment. Your privacy is of upmost importance and therefore, the risk of personal or confidential information being misdirected is not one that we are willing to take. Any forms or surveys available for online submission will not contain any identifiable information.
